Non-Profit Bulk Mailing
Additional Point of Entry (APOE) Form #3623
To authorize us to send a Non-Profit Bulk Mailing on your behalf the USPS requires that we have 2 ORIGINAL documents:
*Item 1 must be how your organization is listed on your bulk mail permit (not a variation).
*Item 7 the authorization number is a separate number that references your nonprofit status. It is not your permit number. You can find it on the letter sent to you by the USPS Nonprofit Authorization Center when you were first approved. Or, by calling the post office where your bulk permit was issued, they can look up what your nonprofit authorization number is to fill out the form.
*Field 8 should read "Vista, CA 92084"
*Fields 1-7 and 11-13 must be filled out
- a letter of authorization printed on your organization’s letterhead (sample below)
Dear Postmaster:The enclosed Application Mailing Office PS Form 3623 is being submitted, requesting authorization to mail at the Nonprofit Standard Mail rates out of the Vista, CA mailing office.
Additionally, I request that the post office supply the ghost number and the authorization number to Outreach, Inc for use in our mailing.
